Refund Request Form - all requests for a refund must begin with this form.

Volunteer Incentive Program Form - Collect your volunteer hours over the season then submit your form to be eligible for our Incentive Program prizes.

Sponsorship Form - The HLA is actively recruiting sponsors for all House League teams. Note to our members: bringing in a sponsor entitles you to participate in our V.I.P. (Volunteer Incentive Program).

Volunteer Registration Form - The HLA only succeeds based on the strength of it's volunteer base. Become a bigger part of this growing organization and offer a few hours to the league.

OLA Release Form - The Ontario Lacrosse Association has a form which is required to be filled out when applying for a player's release.  There is also a procedure to follow. Click on the hi-lited to download.  The H.L.A.
